Patron Dispute Form

In accordance with Gun Lake Tribal Gaming Ordinance and Gun Lake Tribal Gaming Commission Rules and Regulations, all patrons of the Gun Lake Casino are entitled to file a patron dispute with the Gun Lake Tribal Gaming Commission if they find that they have been treated unjustly with the regard to the integrity and fairness of gaming within this licensed gaming establishment. The Tribal Gaming Commission does not investigate patron disputes that are outside the scope of gaming activities. Any patron dispute that is determined to be outside the scope of gaming will be forwarded to appropriate agencies and notification will be sent to the patron of this action.

All patron disputes should be filed within five calendar days from the date of the incident in question. Any dispute that is received after this five-day threshold will be investigated, however, the Commission may be limited in their investigative abilities. A full investigation will be conducted within 30 calendar days from filing. The findings of the patron dispute investigation will be reviewed by the Tribal Gaming Commission’s Executive Director and a determination as to the merit of the dispute will be made. A letter of determination with the results of the investigation will be mailed to the residence you provide below. If you are dissatisfied with the Executive Director’s decision, you are entitled to file a written appeal requesting a hearing with the Gaming Commission Board within 14 calendar days from the date of receipt of the Executive Director’s determination. If a hearing is granted, the burden of proof is on you to demonstrate that you are entitled to relief.

All investigations are executed by the Gun Lake Tribal Gaming Commission, not Gun Lake Casino, and are conducted in accordance with §4 of the Gun Lake Tribal Gaming Commission Rules and Regulations.
